How to Use DASIE...

 
 
  1. 1.To use your DASIE, tape each end to the surface of a table to prevent rotation with the side you plan to incise facing upward (see picture above).  The DASIE can be turned over in order to use multiple surfaces for cutting and suturing practice.

  2. 2.The DASIE can be used to practice draping and application of a laparotomy sheet but can also be used as is.

  3. 3.Once your DASIE is stable, incise the DASIE as you would tissues, identifying and gradually incising each layer:

    i.  First cut through the skin layer (faux-suede), below you will encounter end-on vessels that mimic subcutaneous / dermal bleeders.  Practice clamping and ligating vessels.
    ii. Then incise the subcutaneous tissues (foam)
    iii.  Finally, elevate the subcutaneous tissues to identify the abdominal wall, grasp the wall, stab and then incise.  The abdominal wall mimics a slight paramedian incision where the external rectus fascia, rectus abdominal muscle and internal fascia peritoneum  are seen as separate layers.
    iv. Within the abdominal cavity is an intestine-like tube that can be used to practice enterotomy and closure as well as resection and anastomosis.

  4. 4.Each abdominal wall layer can then be closed individually:
                * the abdominal wall (full thickness or external fascia only)
                * the subcutaneous fat
                * the intradermal tissue (if desired)
                * the skin


Practice any suture pattern on any of the layers.  The deeper subcutaneous layer is used to practice burying the knot.  If an area becomes less amenable to suture, turn the DASIE over and repeat the above steps in a different area.


With time, the DASIE layers are liable to de-lamination because some materials are less responsive to glue.  These materials were specifically chosen to mimic normal tissues during incising and suturing.  The learning opportunity provided by DASIE is not compromised by de-lamination.  DASIE is a work in progress.  We continually strive to improve the model and will continue to improve the materials in order to achieve the best results possible.
